1、布托啡諾超前鎮(zhèn)痛對(duì)羅哌卡因臂叢阻滯效果影響的臨床評(píng)價(jià)王德明張?jiān)艑O永興何旭[作者單位：266071山東省青島市，解放軍第401醫(yī)院全軍手外科中心麻醉科（王德明、張?jiān)?、何旭）；首都醫(yī)科大學(xué)附屬北京同仁醫(yī)院麻醉科（孫永興）)摘要]目的評(píng)價(jià)布托啡諾超前鎮(zhèn)痛對(duì)羅哌卡因臂叢阻滯效果的影響。方法選擇80例擬行前臂與手部手術(shù)的成年男性患者，ASAⅠ-Ⅱ級(jí)。隨機(jī)分為四組（n=20），每組均用4mg/ml羅哌卡因40ml作腋路臂叢神經(jīng)阻滯，各組局麻藥均不加腎上腺素。A組：對(duì)照組，臂叢神經(jīng)阻滯前5min靜注安慰劑。B組、C組、D組分別在臂
2、叢神經(jīng)阻滯前5min靜注布托啡諾0.5mg、1mg、2mg。在麻醉后5、10、15、20、30、45、60min對(duì)麻醉質(zhì)量進(jìn)行評(píng)價(jià),記錄麻醉起效時(shí)間、麻醉維持時(shí)間、手術(shù)時(shí)間、圍術(shù)期各時(shí)間點(diǎn)的疼痛評(píng)分，疼痛程度采用“視覺模擬評(píng)分法”（VAS）評(píng)定、不良反應(yīng)發(fā)生率。結(jié)果麻醉維持時(shí)間A組略短于B組，但兩組比較差異無顯著性（P>0.05）；C組、D組明顯長于A組、B組（P<0.01）。術(shù)后8h、12h、24h時(shí)疼痛評(píng)分（VAS），A組、B組高于C組、D組（P<0.01）。C組、D組輔助用藥量明顯低于A組和B組。結(jié)論布托啡諾超前鎮(zhèn)
