2、etal【摘要】目的應(yīng)用急性生理與慢性健康狀況評分(APACHE)I1評分系統(tǒng)對急診內(nèi)使用無創(chuàng)通氣治療的患者進(jìn)行病情頇后分析。方法回顧性分析2010年1月一2012年7月急診室使用無創(chuàng)通氣的急危重癥患者的APACHElI評分結(jié)果，根據(jù)患者的死亡和接受氣管插管作為預(yù)后不良，成功脫機(jī)為預(yù)后良好，比較預(yù)后良好和預(yù)后不良患者臨床特征、病因和APACHEH分值間的差異，評價APACHEⅡ和預(yù)后的相關(guān)性。結(jié)果62例入選病例中男34例．女28例，根據(jù)BIPAP治療后病情轉(zhuǎn)歸情況分為：預(yù)后良好組32例，預(yù)后不良組3O例。預(yù)后不良組APACHE
3、H評分分值明顯高于預(yù)后良好組(P<0．01)，單因素回歸分析發(fā)現(xiàn)APACHE11分值與患者預(yù)后不良的發(fā)生率呈正相關(guān)(OR：1．112，P=0．029)，APACHEⅡ分值可預(yù)測患者預(yù)后不良，曲線下面積為0．658(P<0．02)，Cutof值為>17，其診斷靈敏度為66．67(47．2～82．7)，特異度為59．38(40．6—76．3)。陽性預(yù)測值60．6(42．177．1)，陰性預(yù)測值65．5(45．7～82．1)。結(jié)論APACHEⅡ評分對急診室內(nèi)使用無創(chuàng)通氣的不同病因的患者具有一定的預(yù)后預(yù)測作用?！娟P(guān)鍵詞】APACHEⅡ
