1、放射學(xué)實(shí)踐2014年8月第29卷第8期RadiolPractice,Aug2014,Vol29,No.8893中樞神經(jīng)影像學(xué)MRI動(dòng)態(tài)增強(qiáng)掃描定量參數(shù)對腦膠質(zhì)瘤分級診斷價(jià)值的研究范兵,杜華睿,王霄英,許玉峰,李沙,張玨【摘要】目的:探討MRI動(dòng)態(tài)增強(qiáng)掃描定量參數(shù)對腦膠質(zhì)瘤病理分級的診斷價(jià)值。方法:經(jīng)手術(shù)病理證實(shí)的腦膠質(zhì)瘤患者19例,其中高級別膠質(zhì)瘤(HGG)12例,低級別膠質(zhì)瘤(LGG)7例。所有患者均行常規(guī)MRI平掃及MRI動(dòng)態(tài)增強(qiáng)(DCE)掃描。利用北京大學(xué)前沿交叉學(xué)科研究院開發(fā)的Ktrans計(jì)算系統(tǒng),分別測量顱內(nèi)占位性病變實(shí)質(zhì)區(qū)及瘤周水腫區(qū)的Ktrans值和Ve值
2、。不同級別膠質(zhì)瘤Ktrans值的比較采用Mann-WhitneyU檢驗(yàn)。采用受試者工作特征曲線(ROC)對Ktrans和Ve值進(jìn)行分析。結(jié)果:HGG及LGG組的Ktrans值分別為(0.24±0.17)和(0.07±0.04)rain~,差異有統(tǒng)計(jì)學(xué)意義(£一2.625,P一0.018);Ve分別為0.92±0.15和0.50±0.25,差異有統(tǒng)計(jì)學(xué)意義(一4.613,P(O.001);Ktrans以0.12min為閾值,診斷敏感度為83,特異度為86;Ve以0.88為閩值,敏感度為83,特異度為85。結(jié)論:采用Ktrans值結(jié)合常規(guī)MRI序列,對于膠質(zhì)瘤的分級診斷具有
