1、椎體后凸成形術(shù)治療骨質(zhì)疏松性脊柱壓縮骨折作者:海涌陳志明彭軍 吳繼功馬華松鄒德威【關(guān)鍵詞】骨質(zhì)疏松摘要:[目的]分析椎體后凸成形術(shù)治療骨質(zhì)疏松性椎體壓縮骨折的臨床和影像學(xué)結(jié)果,評價其臨床效果。[方法]本組43例骨質(zhì)疏松性椎體壓縮骨折均接受經(jīng)皮椎體球囊擴(kuò)張后凸成形術(shù)。其中男19例,女24例;年齡56~85歲,平均682歲。本組共61個椎體骨折,其中單椎體26例,兩椎體骨折11例,三椎體骨折4例,四椎體骨折2例。椎體骨折部位T7~L5。術(shù)后對患者的疼痛、日常功能以及影像學(xué)結(jié)果進(jìn)行了分析。[結(jié)果]所有患者隨訪1a以上,平均188個月(12~36個月)。平均VAS評分由
2、術(shù)前86到術(shù)后23和最終隨訪27(P<0001),Oswestry評分由術(shù)前55到術(shù)后30和最終隨訪35(P<001)。手術(shù)椎體前、中柱平均高度由術(shù)前的1220mm到術(shù)后的2538mm和最終隨訪2636mm。脊柱矢狀位后凸畸形改善平均99°(43~22°),隨訪丟失平均16°(08~17°)。4例發(fā)生骨水泥滲漏,但無嚴(yán)重并發(fā)癥發(fā)生。最終隨訪時有3例患者發(fā)生3個臨近節(jié)段椎體壓縮骨折。[結(jié)論]球囊擴(kuò)張椎體后凸成形術(shù)可有效恢復(fù)骨質(zhì)疏松性骨折椎體的高度、迅速緩解疼痛、改善病人的功能,明顯減少骨水泥的滲漏率,是一種安全、有效的治療方法?! £P(guān)鍵詞:骨質(zhì)疏松;椎體壓縮骨折;
