1、·18·全科醫(yī)學(xué)臨床與教育2014年1月第12卷第1期ClinicalEducationofGenerMPracticeJan.2014,Vo1.12,No.1著·耐亞胺培南肺炎克雷伯菌分子流行病學(xué)及耐藥傳播機(jī)制研究王步補(bǔ)阮戰(zhàn)偉項(xiàng)小嬋洪亮王曉峰李強(qiáng)『摘要1目的探討臨床收集亞胺培南耐藥肺炎克雷伯菌的耐藥分子機(jī)制和克隆流行情況。方法采用K—B紙片法進(jìn)行藥敏實(shí)驗(yàn),多位點(diǎn)序列分型(MLST)進(jìn)行臨床菌株克隆分型,濾膜接合法進(jìn)行質(zhì)粒接合實(shí)驗(yàn),PCR擴(kuò)增篩選菌株的常見耐藥基因和其周圍序列結(jié)構(gòu)。結(jié)果亞胺培南耐藥肺炎克雷伯菌對青霉素類、頭孢菌素類、含抑制劑B一內(nèi)酰胺類、氨基糖苷類等藥物均顯示出很
2、強(qiáng)的多重耐藥性,MLST分型顯示耐藥克隆具有明顯的流行特征,l3株攜帶bla㈣基因的流行克隆均為ST11型。這13株細(xì)菌亞胺培南耐藥性均由可接合性質(zhì)粒介導(dǎo),并且同時介導(dǎo)和轉(zhuǎn)移了編碼CTX—M型、TEM型或DHA型的B一內(nèi)酰胺酶基因。Tn4401一Tn3復(fù)合轉(zhuǎn)座子或其變體2介導(dǎo)了blaKPc一2基因的轉(zhuǎn)移。結(jié)論臨床肺炎克雷伯菌亞胺培南耐藥的主要原因是產(chǎn)KPC一2酶菌株的克隆播散.同時可接合質(zhì)粒造成的耐藥基因快速水平轉(zhuǎn)移也起到了重要作用。f關(guān)鍵詞1亞胺培南耐藥;多位點(diǎn)序列分型;質(zhì)粒接合;KPC酶Molecularepidemiologyanddrug-resistancetransf
