1、臨床醫(yī)學(xué)論文?嚴(yán)重后足損傷后足重建的臨床硏究【摘要】探討嚴(yán)重后足損傷后，通過后足重建恢復(fù)后足外形及功能的方法和臨床效果?！卜椒ā硨?duì)6例嚴(yán)重后足損傷，進(jìn)行后足重建術(shù)。其中，男4例，女2例。受傷原因?yàn)楦咛帀嬄鋫?例，車禍傷1例。6例均有距骨及跟骨粉碎性骨折，距下、距舟、跟骰關(guān)節(jié)不同程度的脫位，跟骨骨折Sanders分型均為IV型；4例伴有后足前、內(nèi)側(cè)或足底跖側(cè)皮膚挫裂傷，2例為后足前、內(nèi)側(cè)皮膚挫裂傷，另2例為足底跖側(cè)皮膚挫裂傷，其中1例為后足前、內(nèi)側(cè)皮膚撕脫傷伴皮膚壞死缺損，1例為足底跖側(cè)皮膚挫裂傷伴皮膚壞死缺損骨外露，按Gustilo分型：II型2例，皿型A度1例，HI型B度1例?！步Y(jié)果〕2
2、例無皮膚挫裂傷者手術(shù)切口I期愈合；4例伴有皮膚挫裂傷中1例手術(shù)切口滲出、裂開，經(jīng)換藥于術(shù)后4周愈合，余3例手術(shù)切口均I期愈合；4例伴有后足前、內(nèi)側(cè)或足底跖側(cè)皮膚挫裂傷者清創(chuàng)后2例傷口I期愈合，1例后足前、內(nèi)側(cè)皮膚撕脫傷伴皮膚壞死缺損、1例后足足底跖側(cè)皮膚挫裂傷伴皮膚壞死缺損骨外露，經(jīng)換藥形成肉芽創(chuàng)面新鮮后游離植皮分別于術(shù)后6、10周逐漸愈合。依據(jù)美國足踝肯科協(xié)會(huì)的足部評(píng)分標(biāo)準(zhǔn)(A0FAS)評(píng)分評(píng)價(jià)術(shù)后功能6例中：評(píng)分最低68分，最高91分。優(yōu)1例，良3例，可2例?！步Y(jié)論〕嚴(yán)重后足損傷后，通過后足重建術(shù)，可以恢復(fù)后足的外形，最大限度的保留后足功能，是治療嚴(yán)重后足損傷的有效方法。【關(guān)鍵詞】嚴(yán)重
