1、吻合血管皮瓣移植修復前臂和手部復雜性軟組織缺損作者：甘干達，唐繼全，扈克治，陶智剛，羅平【摘要】目的探討吻合血管皮瓣移植修復前臂和手部復雜軟組織缺損的臨床效果。方法對17例前臂和手部復雜軟組織缺損采用股前外側(cè)皮瓣、胸臍皮瓣、小腿內(nèi)側(cè)與股前外側(cè)皮瓣的串聯(lián)皮瓣游離移植修復創(chuàng)面。創(chuàng)面面積11cm×8cm～26cm×9cm，皮瓣切取面積13cm×9cm～27cm×10cm。結(jié)果移植皮瓣全部成活;8例皮瓣臃腫，需再次手術(shù)整形;4例骨缺損在皮瓣成活后半年施行自體髂骨植骨鋼板內(nèi)固定，骨折愈合，前臂的旋轉(zhuǎn)功能和手部
2、功能恢復滿意;3例肌腱缺損者術(shù)后3～5個月用健側(cè)掌長肌腱或手屈指淺肌腱移植修復，手部功能獲得部分恢復;2例尺、橈動脈嚴重損傷，合并重度皮膚肌肉組織缺損者手部功能獲得部分恢復，其余患者前臂旋轉(zhuǎn)功能和手部功能恢復滿意。結(jié)論吻合血管皮瓣移植能有效地修復前臂和手部復雜軟組織缺損，控制創(chuàng)面感染，有助于骨折愈合和手部功能的恢復?！娟P(guān)鍵詞】軟組織缺損;皮瓣移植;組織修復;前臂;手部 【Abstract】ObjectiveTodiscusstheclinicaleffectofvascularizedflaptr

8、長期換藥，培養(yǎng)肉芽后植皮;胸、腹部帶蒂皮瓣修復等。這些方法耗時長，病人痛苦，不易接受，且對手部功能恢復不利。我院2003年10月～2009年2月應用游離皮瓣移植方法修復前臂和手部復雜性軟組織缺損17例，獲得滿意療效?！ ∨R床資料 1一般資料 本組17例，男性12例，女性5例;年齡25～47歲，平均37歲。致傷原因：機器絞傷9例，壓砸傷5例，道路交通傷2例，電鋸傷1例。損傷部位及受傷程度：尺、橈動脈嚴重損傷合并重度皮膚肌肉組織缺損2例(圖1)，前臂屈側(cè)大面積皮膚肌肉
